Insurance *

We accept most out of network PPOs, HSA, FSA and compensation from Injury of Car Accidents. As you know, every plan is different. In order to best serve you we can verify your insurance benefits before you arrive. We will check your benefits with the company to find out whether your insurance plan covers acupuncture or not, how much deductible you should pay, how many times you can visit us and etc. You can email us your insurance company name, policy/ID number, your full name and date of birth with a copy of your insurance card. We will check out your benefits and contact you as soon as the benefits are established.

*We will need to charge $7 administrative fees if your insurance company does not pay for the date of service we help you claim.

Fees for Services of non-insurance payments *

• First time consultation and physical examination $100 

• Acupuncture $120

• Moxibustion $120

• Cupping $40 per 15 mins with 15 min increments. (Does not include free first-time consultation and physical examination)

• Manual therapy: Acupressure/Muscle & fascia alignment therapy $60 per 15 mins with 15 min increments.

• Comprehensive treatment: Acupuncture with moxibustion, and cupping $240

• Ultimate treatment: Acupuncture with moxibustion,cupping and manual therapy $300

• Herbal formula prescribing consultation visit $80; Herbal formula prescription prices vary.

*No-Show Policy: As a courtesy to our office as well as to those patients who are waiting to schedule with the doctors, please give us at least 24 hours notice. If you do not cancel or reschedule your appointment with at least 24 hours notice, we may assess a $200 “no-show” service charge to your account. This “no-show charge” is not reimbursable by your insurance company. You will be billed directly for it.
